1. EPYSA RG-6 Registration Form (all players)

2. DASC Player Information Form (all players)

3. Parent/Athlete Concussion Form A (all players)

4. Code of Conduct Forms (all players, parents, and coaches)

5. 1x1 headshot photo of the player - may be emailed to:
[email protected] (U9 and older boys travel players)
[email protected] (U9 and older girls travel players)

6. Proof of age - Official Birth Certificate copy or Passport copy.
(U9 and older travel players never registered in Dillsburg)
